Street Outreach

NMCAA | Street Outreach

The Street Outreach Program provides essential services crucial for reaching unsheltered individuals experiencing homelessness. Additionally, this program is dedicated to connecting people in need with necessary resources. Furthermore, it focuses on aiding individuals in finding housing, emergency shelter, and critical services like urgent or non-urgent facility-based care. Moreover, the Street Outreach Program offers two key services—Engagement and Case Management. As a result, these services work together to provide comprehensive support to those in need. In summary, the program plays a vital role in helping individuals transition from homelessness to stable living conditions. Therefore, by offering these services, the Street Outreach Program ensures that unsheltered individuals have access to the essential care and resources they require.

By offering compassionate and comprehensive support, we strive to make a positive difference in the lives of those we serve. Supporting those Experiencing Homelessness

The Northwest Michigan Community Action Agency is here to support you. Moreover, if you or someone you know is experiencing homelessness or at risk of losing housing, please know that you are not alone. Additionally, we understand the challenges you may be facing, and we are dedicated to being a guiding light during these difficult times. Therefore, reach out to us—we’re here to help you find the resources and support you need to regain stability and move forward. Ultimately, your well-being matters to us, and together, we can work toward a brighter future.

NMCAA's Homeless Prevention team works tirelessly to help those experiencing homelessness through our Street Outreach Program.

Engagement

During the engagement process, NMCAA focuses on locating, identifying and building relationships with people experiencing homelessness, with the purpose of providing immediate support. This happens through intervention and/or connection with the homeless assistance programs and/or mainstream social services and housing programs.

As the outreach team assesses individuals and situations for eligibility, they may receive crisis counseling, which further addresses urgent physical needs such as needing meals, blankets, clothes, or toiletries. Furthermore, this actively connects and provides information as well as referrals to programs targeted to people living in homelessness. NMCAA may be able to provide additional services by connecting individuals with emergency shelter, transitional housing, community-based services, permanent supportive housing, and/or rapid re-housing programs.
