How Homeless Prevention Works

Above all, the Homeless Prevention Program’s team is dedicated to the commitment of making homelessness rare, brief, and non-recurring in every community across our 10-county service region. In addition to this, we continuously uphold this commitment to the best of our abilities through partnerships with other agencies such as Northwest Michigan Coalition to End Homeless (NWCEH), the Michigan Balance of State Continuum of Care (MIBOSCOC), and the Northeast Michigan Coalition Against Homelessness (NEMCAH). The Homeless Prevention program actively offers the necessary assistance to prevent individuals or families from moving into an emergency shelter or becoming homeless. First and foremost, this program has been designed to help individuals and families regain stability. In fact, this may involve assisting with maintaining current permanent housing or supporting a move into other permanent housing to achieve stability.

Street Outreach

The Street Outreach Program directly provides essential services that are necessary to reach unsheltered people experiencing homelessness. This program is especially dedicated to connecting people in need. Its primary goal is to aid in securing housing, accessing emergency shelter, and obtaining critical services such as urgent or non-facility-based care. Moreover, the Street Outreach program offers two major services: Engagement and Case Management.

Rapid Re-Housing

Learn More about Rapid Rehousing

The Rapid Re-Housing program actively assists individuals and families who are literally homeless or in emergency shelters by solving the practical and immediate challenges of obtaining permanent housing. Additionally, this program reduces the amount of time they experience homelessness, helps avoid a near-term return to homelessness, and enables them to achieve long-term housing stability.

Coordinated Entry

Learn More about Coordinated Entry

Coordinated Entry provides a system that allows for coordinated access into our local homeless services, as well as coordinated movement within and towards exiting from the system. This approach significantly increases the efficiency of our homeless assistance system by standardizing access to homeless programs, services, and coordinating referrals. 

Supportive Services for Veteran Families

Learn More about Supportive Services for Veteran Families

The Supportive Services for Veteran Families (SSVF) Program actively supports low-income veteran families residing in or transitioning to permanent housing. Moreover, SSVF grants are awarded to select private non-profit organizations to assist these veteran families in achieving housing stability.
